- Save 5% with an Annual Membership. The Annual Membership discount is non-refundable once services are rendered.
- You can use your HSA to pay for DPC (up to $150/month), and your DPC costs now count toward your deductible if you’re on a high-deductible health plan.
- You’re never locked into a contract—cancel anytime if your needs change.
- There is a one-time registration fee of $75 per person or a $200 family maximum.
- ACH Advantage Policy: Credit and debit cards are subject to a 3.5% administrative and technology fee per transaction. This fee is waived for members using ACH.
- Controlled Medication Management Policy: Membership involving controlled substances carry a supplemental monthly fee.
- Re-enrollment requires a $100 fee and is subject to the waitlist.
- Unfortunately we're unable to accept patients with Colorado Medicaid.
